Should I give him an ultimatum? I really need help, so confused..?

I don't think its a bad thing that you want a choice, I believe it shows you actually care. If you both love each other I believe you should give him the ultimatum, if he truly loves you he would be willing to answer your question, all this time you have been together and have been faithful to each other.
